We Welcome New Patients

We’re always accepting new patients. Many of our patients are referred to our practice by general dentists. However, a referral is not required and we are proud that many patients are referred to us by friends and family. You will have the opportunity to meet with our team and get to know Dr. Andrew Samuel. We are committed to educating you about your dental needs and will answer any questions you may have concerning your treatment.

To provide you with the most efficient new patient experience, please download and complete the forms below and bring them to your first appointment. Our team will take comprehensive records and address any concerns or questions you may have. If you have any problems downloading the forms, please contact our office and we will gladly send them to you.

Your First Visit

Your first visit with us may include:

  • Comprehensive oral examination
  • Full mouth digital X-rays and/or 3-D scans (If one is not available or taken within the last year)
  • Oral cancer screening
  • One-on-one consultation with our dentist to discuss your needs, concerns, goals, and treatment options

During your visit, we will answer any and all questions about your oral health. We will provide a detailed financial consultation to ensure your treatments are timed effectively and affordably

Insurance and Billing

We can help you maximize your benefits. We recommend that you contact our office prior to your appointment to provide us with insurance information for verification of benefits. We accept most major dental insurance and will file most claims for you as a courtesy. We will provide you with an explanation of your dental benefits and financial responsibilities up front, and work with you to ensure financing if needed.

Financing Options

We understand that periodontal and implant treatment is an investment in improving the quality of your life. Our front office team will help you obtain the maximum benefits available through your dental insurance plans. We are a participating provider for Delta Dental plans. Our office works with all dental insurance carriers with out-of-network benefits. Additionally, to make the total cost of treatment more affordable on any budget, we offer CareCredit℠ patient financing. This program works just like a credit card that is available for medical and dental procedures, and has low interest rates with flexible repayment terms. Our administrative team will be happy to assist you with any insurance or financing information.

Payment Options

Financial options are discussed during the initial visit. Dr. Samuel and his staff are committed to providing excellent dental care and will guide patients in choosing the best payment option for their individual needs. We accept cash, check, Visa, MasterCard and Discover.

Treatment Financing

We work with Care Credit Health Care Financing to help our patients obtain the treatment they need by extending payments to fit many budgets.

Please understand that your insurance is a contract between your employer and a dental insurance company. Benefits received are based on the terms of the contract negotiated between your employer and the dental insurance company and not our dental office

The goal of most dental insurance policies is to provide only basic care for specific dental services. The benefits that you receive from the dental insurance company for your current insurance plan have nothing to do with you achieving a high-quality, complete result.

Your benefits are decided between your employer and the insurance company. Many needed services may not be covered. Please remember that you are ultimately responsible for all expenses incurred. We urge you to read the policy so that you are fully aware of coverage and any limitations of the benefits provided. Our staff will be happy to assist you with any insurance matter.
